Long Term Condition (LTC) Review System
This page will tell you all you need to know about our Long Term Condition Invite System.
Each year, we invite all eligible patients for an annual long term condition review. This is for a range of conditions as listed below:
- Cardiovascular/Heart Disease including Atrial Fibrillation, Chronic Heart Disease, Heart Failure & Peripheral Arterial Disease
- Chronic Obstructive Pulmonary Disease (COPD)
- Chronic Kidney Disease (CKD)
- Coeliac Disease
- Hypertension (High Blood Pressure)
- Hypothyroidism / Hyperthyroidism
- Learning Disability
- Mental Health
- Post-Bariatric Surgery
- Stroke and Transient Ischaemic Attacks (TIAs)
We invite all our patients in their birth month for their annual review, either by letter or by SMS (text message) if we have your consent. An annual review is required for all patients with a long term condition. We strongly encourage you to attend the surgery when you receive your invitation from us.
The purpose of an annual review is to ensure patients are taking the right medication and are receiving the best possible care for their condition(s). This is done by conducting the most appropriate tests and reviewing your medication.
Please note: Not all long term conditions are reviewed by Doctors. Many of our annual reviews are more appropriately carried out by our Nursing Team.
For many conditions, your review will only be one appointment, which will include a blood test and a blood pressure check, carried out by one of our Phlebotomists or Healthcare Assistants (HCA).
For any dementia reviews, these will be carried out by one of our doctors. A member of our admin team will be in touch with you to book this appointment during your birth month.
Our mental health checks are currently being carried out by practitioners at Thrive Tribe. Don’t worry - your appointment will still be here at the surgery and you will receive an invite during your birth month to book your appointment.
